What is an upper GI endoscopy?
An upper GI endoscopy, or gastroscopy, is a test in which a thin flexible camera is passed through the mouth to examine the oesophagus, stomach and the first part of the small intestine. It takes five to ten minutes, is done with a throat spray or light sedation, and biopsies can be taken painlessly if needed. Most people go home within an hour or two of the test.
When it is needed
When an upper GI endoscopy is advised
Endoscopy is the test that settles what is actually happening in the upper digestive tract, rather than treating symptoms blindly.
Upper GI endoscopy is the first step towards most advanced and therapeutic endoscopy. A lesion found at endoscopy may go on to EMR or ESD; a suspected motility problem goes to manometry and, if achalasia is confirmed, to POEM.
The procedure
What happens on the day
- Fasting — nothing to eat for six hours and nothing to drink for two hours before the test
- Medication — diabetes and blood-thinning medication is adjusted beforehand on advice
- Throat spray or sedation — a local spray, or light sedation if you prefer to sleep through it
- The test — five to ten minutes; you can breathe normally throughout
- Biopsies — small painless samples if needed — you will not feel them being taken
- Afterwards — a report the same day; eat and drink once the throat spray has worn off
What a good endoscopy adds
The value of an endoscopy is not only in the images but in how carefully the stomach is examined. Early gastric and oesophageal cancers are subtle and are missed on a hurried examination.
Image-enhanced and magnifying endoscopy is used here to inspect suspicious areas, which is what allows early lesions to be picked up while they can still be treated endoscopically.
A normal endoscopy is a useful result. It rules out ulcer, cancer and inflammation, and moves the conversation to the conditions that do not show on a camera.

Frequently asked questions
Is an upper GI endoscopy painful?
It is uncomfortable rather than painful, and only briefly. Most people manage easily with a throat spray alone; if you would rather sleep through it, light sedation is available and you will remember nothing of the test. Biopsies are painless.
How long does it take and when can I eat?
The test itself takes five to ten minutes. With a throat spray you can eat and drink once the numbness wears off, usually after about half an hour. With sedation you will be observed for an hour or so and will need someone to accompany you home.
Do I need to stop my medication before the endoscopy?
Tell the clinic about blood thinners, diabetes medication and iron tablets when you book. Blood-thinning and diabetes medication often need adjusting for the day, and iron tablets are usually stopped a few days beforehand because they stain the stomach lining.
