Dr Surakshith T K is a Senior Consultant Gastroenterologist, Hepatologist and Therapeutic Endoscopist in New Delhi, with over 15 years of experience in the diagnosis and treatment of gastrointestinal, liver and pancreaticobiliary disorders.

Advanced & third-space endoscopy
Modern endoscopy has evolved far beyond diagnostic gastroscopy and colonoscopy.
Third-space endoscopy is an advanced group of minimally invasive techniques in which the endoscopist works within the layers of the gastrointestinal wall to treat conditions that previously often required surgery.
My practice includes:
- POEM (Peroral Endoscopic Myotomy) for achalasia and selected esophageal motility disorders
- ESD (Endoscopic Submucosal Dissection) for early gastrointestinal neoplasia
- EMR (Endoscopic Mucosal Resection) for selected precancerous and early cancerous lesions
- EFTR (Endoscopic Full-Thickness Resection) for selected difficult gastrointestinal lesions
- Advanced EUS-guided diagnostic and therapeutic procedures
- Complex ERCP, including pancreaticobiliary interventions
- Advanced imaging techniques including image-enhanced and magnifying endoscopy
I have performed 175+ POEM procedures and 250+ third-space endoscopic procedures, including POEM, ESD and EFTR.
My aim is to offer patients appropriately selected, minimally invasive treatment while combining advanced endoscopic expertise with careful clinical decision-making.

International training in advanced endoscopy
I have undertaken advanced training in ESD, image-enhanced endoscopy and therapeutic endoscopy in Japan, including training at:

Jichi Medical University, Japan
Advanced training in endoscopic submucosal dissection (ESD) and image-enhanced endoscopy (IEE) under the guidance of leading Japanese experts, with Professor Hironori Yamamoto, Division of Gastroenterology, Jichi Medical University Hospital.

Osaka International Cancer Institute, Osaka, Japan
Advanced exposure to ESD and image-enhanced/magnifying endoscopy, with a focus on the endoscopic diagnosis and treatment of early gastrointestinal neoplasia, with Professor Noriya Uedo, Department of Gastrointestinal Oncology.
Training in Japan has been an important part of my approach to advanced endoscopy, particularly the emphasis on meticulous lesion assessment, precise endoscopic diagnosis and organ-preserving treatment.

Advanced technology is only useful when it is applied to the right patient
My approach is based on three principles.
Correct diagnosis first
Before recommending an advanced procedure, I review the clinical history, previous investigations, imaging, endoscopy and relevant physiological testing.
Choose the least invasive effective treatment
When an endoscopic treatment can provide an effective alternative to surgery, it should be considered where appropriate. At the same time, I believe patients should be advised against procedures that are unlikely to benefit them.
Long-term outcomes matter
The objective is not simply to complete a procedure. It is to achieve a meaningful clinical outcome while anticipating complications, recurrence, nutritional issues and long-term follow-up requirements.
For complex cases, I also provide second opinions and treatment planning based on existing medical records and investigations.

What is third-space endoscopy?
Third-space endoscopy is an advanced group of minimally invasive techniques in which the endoscopist works within the layers of the gastrointestinal wall to treat conditions that previously often required surgery. It includes POEM for achalasia, ESD for early gastrointestinal neoplasia and EFTR for selected difficult lesions. Dr Surakshith has performed more than 250 third-space endoscopic procedures, including 175+ POEM.
Does Dr Surakshith perform EUS and ERCP?
Yes. Complex EUS and ERCP are a central part of his practice, with more than 10,000 ERCP and 5,000 EUS procedures performed, of which over 2,500 are advanced or interventional. This covers EUS for pancreaticobiliary disease, gastrointestinal lesions, tissue acquisition and EUS-guided intervention, and ERCP for biliary obstruction, bile duct stones, strictures and other therapeutic pancreaticobiliary indications.
Where did he train in ESD and image-enhanced endoscopy?
In Japan. He has trained in endoscopic submucosal dissection and image-enhanced endoscopy at Jichi Medical University, Japan, and in ESD and image-enhanced/magnifying endoscopy for early gastrointestinal neoplasia at the Osaka International Cancer Institute, Osaka.
