PERORAL ENDOSCOPIC MYOTOMY

The endoscope is used to perform the POEM procedure. No incisions are made in the chest or abdomen. The endoscope (a thin flexible tube) is inserted into the esophagus through the patient's mouth. An incision is made in the lining (mucosa) of the inside of the esophagus with a blade on the end of the scope, and the scope is passed through the space created between this mucosal layer and the muscular layers of the esophagus. The POEM procedure is used to treat achalasia, a swallowing disorder. Achalasia is an esophageal motility disorder in which the esophagus loses its normal ability to move food and liquids into the stomach. Furthermore, the lower esophageal sphincter, a muscular ring that controls food passage from the esophagus to the stomach, does not relax sufficiently to allow food and liquids into the stomach.
